Creating a Birth Plan: More Than Just “Epidural or Not”

A Guide from a St. Louis Doula and St. Louis Birth Photographer

When most families tell me they “have a birth plan,” it often means one thing:
“I want an epidural.”
or
“I don’t want an epidural.”

And while pain management is absolutely important, a birth plan is so much more than that.

As a St. Louis doula and St. Louis birth photographer, I’ve learned that a birth plan isn’t about control. It’s about communication. It’s about clarity. It’s about walking into your birth space feeling informed, supported, and confident.

Birth is unpredictable. But preparation? That’s powerful.


What Is a Birth Plan Really?

A birth plan is not a rigid script.
It is a guide for your care team.

It tells your nurses, providers, partner, and doula:

  • What helps you feel safe

  • What your preferences are

  • What your fears might be

  • How you want to be supported

  • What matters most to you if plans change

It opens the door to conversations long before labor begins.


What Should Be Included in a Birth Plan?

Here are the most important elements I help my St. Louis families think through:

1. Labor Environment Preferences

  • Dim lighting or bright?

  • Music or quiet?

  • Minimal interruptions?

  • Who is in the room?

  • Photography or no photography?

As your St. Louis birth photographer, I also discuss:

  • Are you comfortable with crowning images?

  • Do you want modest coverage?

  • Should I capture procedures or focus only on emotional moments?

These are deeply personal choices — and they matter.


2. Pain Management Preferences

Yes — this is part of it.

But instead of just “epidural or not,” we discuss:

  • Do you want to labor naturally as long as possible?

  • Would you like to try movement, shower, tub, birth ball first?

  • Are you open to IV medications?

  • If you choose an epidural, when would you prefer it?

As a St. Louis doula, my role is never to sway you — but to support you in whatever path feels right.


3. Movement & Positioning

Many families don’t realize they can request:

  • Freedom to move

  • Intermittent monitoring (if low risk)

  • Different pushing positions

  • Side-lying or hands-and-knees pushing

Your body was designed to move in labor. Knowing your options changes everything.


4. Interventions

This is where education really matters.

Your birth plan can address:

  • Induction preferences

  • Breaking of waters

  • Cervical checks

  • Pitocin use

  • Assisted delivery (vacuum/forceps)

  • Cesarean preferences if needed

Birth plans are especially powerful here — because they allow you to say:

“If medically necessary, we understand. But please explain risks, benefits, and alternatives.”

That simple request shifts the energy in the room.


5. Immediate Postpartum Wishes

This is often forgotten — and it’s one of the most important sections.

  • Immediate skin-to-skin

  • Delayed cord clamping

  • Who announces baby’s gender?

  • Golden hour uninterrupted bonding

  • Newborn procedures delayed if possible

  • Feeding preferences

As your St. Louis birth photographer, some of the most sacred images happen during this time. Protecting that space matters.


6. Cesarean Preferences (Just in Case)

Even families planning an unmedicated birth should include this.

You can request:

  • Clear drape

  • Lowered drape

  • Immediate skin-to-skin in OR (if hospital allows)

  • Partner stays with baby

  • Photography in OR (hospital dependent)

A birth plan is not about preventing a C-section.
It’s about preserving your voice if one becomes necessary.


What a Birth Plan Is NOT

It is not:

❌ A contract
❌ A guarantee
❌ A sign you are “high maintenance”
❌ A document to fight your provider

It is a communication tool.

And sometimes, the most important part of a birth plan is this line:

“We understand birth can be unpredictable. Our goal is a healthy baby and healthy mom. Please communicate with us and include us in decisions.”
